What is Acne and How Does it Form?
Acne is a common skin condition that occurs when your hair follicles become clogged with oil (sebum) and dead skin cells. Our skin naturally produces sebum to keep it moisturized and protected. This oily substance normally travels up hair follicles and out through our pores onto the skin's surface. However, several factors can disrupt this process, leading to acne:
- Excess Oil Production: Hormonal fluctuations (especially during puberty, but also during menstrual cycles, pregnancy, or even stress) can lead to an overproduction of sebum, making the skin oilier.
- Retention Hyperkeratosis: This is a crucial, often overlooked, step in acne formation. Normally, our skin cells (keratinocytes) in the lining of the hair follicle shed regularly. However, in retention hyperkeratosis, these cells shed too rapidly and stick together instead of sloughing off. This buildup, combined with excess oil, forms a sticky plug that clogs the hair follicle opening.
- Bacteria: A common skin bacterium, Cutibacterium acnes (formerly P. acnes), thrives in these clogged, oxygen-deprived environments. As these bacteria multiply, they produce byproducts that trigger inflammation and infection within the follicle.
- Inflammation: The body's immune response to the clogged, bacteria-filled follicle leads to redness, swelling, and the formation of visible blemishes.
These factors combine to create different types of acne lesions, from mild whiteheads and blackheads (comedones) to more severe and painful papules, pustules, nodules, and cysts. Acne most commonly appears on the face, forehead, chest, upper back, and shoulders because these areas have the highest concentration of oil glands.

It might seem like a pimple appears overnight, but in reality, acne lesions could have started forming up to three months prior. What you see on the surface has been brewing beneath your skin for weeks. Microscopic clogs, called microcomedones, form first, invisible to the naked eye.
